Important information for students:

2 NOC( for D.Pharmacy)
  • Self attested copies of DMCs
  • Self attested copies of provisional certificates  
  • Original copy of Hospital Training Certificate.
   Nil Applicant can apply through his/her respective institute or  send duly filled application through  Registerd/ Speed post or by submitting the application on the Board Counter.
3 Transcript/WES Verification
  • Photo copy of Diploma certificate 
  • Photocopy of all DMCs
           Nil
  • Application form downloaded from WES Website should be sent through email.
  • Verified documents in sealed envelope will be handed over to applicant  or  person  authorised by the applicant
4 Duplicate Certificate/DMC( In case of Lost)
  • Copy of FIR(in original)
  • Affidavit in original
  • Rs. 350 per certificate ( for issuing 1st time)
  • Rs. 450 per certificate ( for issuing 2nd time)
  • Candidate should apply in prescribed application form, Click to Download Form
  • The line must be added in the affidavit:          "In case the above said  document(s) is traced,I will not misuse the same and will handover the same to concerned authority".
5 Change of Name or any other correction
  • Self attested copy of 10th certificate,
  • Copy of Diploma /DMCs (in original)

6 Provisional Certificate( In case of Closed Institute
  • Self attested copy of Diploma/DMCs
  • Self attested copy of Adhar Card
          Nil Students should apply to Secretary, H.P.Takniki Shiksha Board and send it through email or speed post
7 Re-evaluation   Rs. 350/- per subject
  • Polytechnic students should apply through online mode:
  • Important Instructions to apply for Re-evaluation 

    1. Enter your Roll No.

    2. Enter your Date of Birth

    3.Click on "Login" button

    4. Press "Verify Details" button

    5. Enter valid Mobile Number

    6. Enter Valid email -id ( to get OTP)

    7. Press "OTP verify button"

    8. Now select subject(s) in which you would like to seek re-evaluation 

    9. Payment will be automatically calculated per subject@ Rs.350/- per subject

    10. Go for online payment through Debit card/Credit card/Netbanking etc..

8 Re-checking Payment receipt should be attached with application form Rs. 200/- per subject

 Students should apply in prescribed application form through offline mode 

Application should reach in the Board office within 30 dyas after declaration of result.

9 To see Answer Sheet Payment receipt should be attached with application form Rs. 600/- per subject

Students should apply in prescribed application form through offline mode.

Students can apply directly to Board and the application should reach to Board office within 30 days after declaration of Result

10 Issuing Duplicate registration card Payment receipt  alongwith   original old registration card       Rs. 50/- Students should apply to Secretary, H.P.Takniki Shiksha Board through their respective  institute.
